STATEMENT OF WORK

Task 1

Provide services including assisted and independent living with services to persons who cannot afford to pay for services and who are screened and referred by U-First Healthcare, LLC.

Task 2

Provider shall refer clients to residential care, to and for independent and assisted living facilities providing outpatient, virtual, and concierge medical practice offering primary family health, psychiatrist mental health, behavioral health, therapy, counseling, and other specialty services to support self-care, prevention, fitness, and nutrition AA, and NA, and or any other resources appropriate to the continuum of the care plan. The aftercare plan and referrals shall be documented in the case of records by a case manager.

Task 3

Medical services organizations and the Veteran’s Administration Tucker, GA Tucker, DeKalb, and Gwinnett County, Georgia will be the source of referrals to the program. Certification of medical clearance must be provided by the referring agency for any client referred on a form provided by U-First Healthcare, LLC. Provider shall refer clients to residential care, to and for independent and assisted living facilities providing outpatient, virtual, and concierge medical practice offering primary family health, psychiatrist mental health, behavioral health, therapy, counseling, and other specialty services to support self-care, prevention, fitness, and nutrition AA, and NA, and or any other resources appropriate to the continuum of the care plan. The aftercare plan and referrals shall be documented in the case of records by a case manager.

Task 4

Provider shall maintain a system to identify patient care, recidivists, acute care provider shall refer clients to residential care, to and for independent and assisted living facilities providing outpatient, virtual, and concierge medical practice offering primary family health, psychiatrist mental health, behavioral health, therapy, counseling, and other specialty services to support self-care, prevention, fitness, and nutrition AA, and NA, and or any other resources appropriate to the continuum of the care plan. The aftercare plan and referrals shall be documented in the case of records by a case manager.

Task 5

Provider shall continue to encourage and shall refer clients to residential care, to and for independent and assisted living facilities providing outpatient, virtual, and concierge medical practice offering primary family health, psychiatrist mental health, behavioral health, therapy, counseling, and other specialty services to support self-care, prevention, fitness, and nutrition AA, and NA, and or any other resources appropriate to the continuum of the care plan. The aftercare plan and referrals shall be documented in the case of records by a case manager. In addition to patients who are dually diagnosed with substance abuse and mental health and or NA to provide all clients the in-house opportunity to experience the AA and/or NA programs during their course of treatment at the facility and shall document in-house AA and NA activity and aftercare referrals in the client’s case record.

Task 6

Provider shall maintain consistency and a continuum of care level of ninety (90) percent utilization. Inability to maintain ninety percent utilization could result in a possible reduction in capacity as well as corresponding funds for grants, and funding from strategic partners at the city, county, state, and federal levels as determined by this agency.

Task 7

The clinic will be provided with the software to assist with patient management treatment planning and outcome assessment provided by health care organizations, health care clinics and specifically supporting the Veterans Administration Assisted Living Division that does assessment/referral, management information systems portion to establish and maintain a centralized intake and database for community-based drug treatment programs.

Who Will Benefit From Activities Conducted?

1. Low and moderate-income persons who cannot afford quality health care support services.
2. Victims of substance abuse and mental health disorders, PTSD, and have underdeveloped skills due to the trauma of the experience.
3. Community residents will benefit from the services made available by the business ventures and employment opportunities to the neighboring residents
4. Residents will also benefit from the social services made available by the business ventures and employment opportunities to the neighboring residents.
